We are a multi-disciplinary team directed that is evidence based, and consumer led.

We believe that every individual deserves to live a fulfilling life in the comfort of their chosen environment, and our focus is to ensure our valued participants receive the right level of evidence-based supports that go beyond meeting basic needs to enhancing independence, well-being and improved quality of life.

We primarily support NDIS participants with diverse and often complex care needs including:

  • People with disabilities requiring direct daily living and community supports.
  • Individuals with high-intensity and complex care needs, who benefit from specialised nursing and multidisciplinary care.
  • Participants aged 7 to 65. Who require tailored developmental, behavioural and therapeutic supports.
  • NDIS participants and their families seeking assistance with navigating and implementing their NDIS plans to achieve their personal goals.

  • Our ventilator skilled nurses and fully trained support workers in ventilator management will ensure that you receive the best level of care tailored to your unique needs
  • We provide ventilator care service that allows you to live comfortably, flexibly and safely in your chosen environment.

  • Our focus is aimed at empowering, promoting and maintaining your independence as much as possible, free from the rigid schedules of hospital and other acute communal clinical settings.
  • Participants requiring tube feeds or intravenous nutrition will be fully supported by our skilled nurses and trained support workers to deliver and maintain these specialised feeding regimes in collaboration with Dietician and participants feed plans.
  • Services will extend to management of the stoma, tube and other complications that can arise during this highly specialised nutrition support.

  • Our highly skilled PBS practitioner will provide therapeutic supports (assessment and plans) that can be implemented by the care workers to develop new skills that enable participants to find other ways of communicating.

Our specialist support coordinators will:

  • Work with you, your families and carers to ensure all services can work effectively and cohesively together to help you pursue your goals
  • Establish and understand the complex barriers that affect your access, use and maintenance of your supports
  • Navigate through available resources to overcome barriers and prioritising finding solutions that are urgently required
  • Ensure a consistent delivery of service and access to relevant supports in a crisis or unplanned event.
